AI Appointment Setter Cost: What You Actually Pay
Josh Pocock
8 min read
An AI appointment setter is a voice or SMS agent that answers your leads, qualifies them and books them onto your calendar. Owners pricing one usually compare it against software tools, agencies and a human setter or receptionist.
AI appointment setter cost is the total you pay to turn leads into booked appointments: software or agency fees, phone and texting charges, setup work and your own time. The fair way to compare options is cost per booked appointment and cost per show, not the monthly invoice.
Below are the ways to buy, what drives the price up or down, the costs people forget, and a worked example you can copy with your own numbers.
The ways to buy an AI appointment setter
You can buy this capability in four basic shapes. Each one bills differently, so the sticker prices are hard to compare until you convert them to cost per booking.
DIY software
You sign up for a voice or SMS AI platform and build the agent yourself. Pricing is usually per minute of talk time, per seat, per message or a usage tier, plus separate charges for phone numbers and texting.
The software bill is the small part. The big part is the hours you spend writing prompts, connecting your calendar and CRM, testing calls and fixing what breaks. If you do not count those hours, DIY always looks cheapest.
Done-for-you agency
An agency builds, connects and runs the agent for you. The usual model is a one-time setup fee plus a monthly retainer, and some agencies pass usage (minutes and texts) through at cost or bundle it in.
You pay more in cash and less in time. The questions to ask are who owns the scripts, who tunes the agent after launch, and what happens to your phone numbers if you leave.
Human appointment setter
A human setter is usually paid a base wage plus a bonus per booked appointment, per show or a commission on closed deals. There is no clean public wage figure for "appointment setter," so the closest public benchmark is receptionists.
The BLS Occupational Outlook Handbook lists a median receptionist wage of $18.27 per hour, or $38,010 per year (May 2025). Treat that as a floor for base pay. Setters with sales skills often earn more once bonuses are added, and you still pay payroll taxes, software seats and management time on top.
Hybrid
Many businesses run the AI on first response and booking, then route hand-offs to a person. The cost is the AI line item plus a smaller slice of human time. We break down that split in AI appointment setter vs human appointment setter.
What drives the price
Two businesses can buy the same tool and pay very different amounts. These are the line items that move the number.
- Call minutes and message volume. Per-minute and per-message pricing scales with lead flow. Long qualification calls cost more than short booking calls.
- Channels. Voice only, SMS only or both. Each channel has its own carrier fees and setup.
- Integrations. Calendar, CRM, lead forms and ad platforms. Every connection is build time, and custom ones cost more than native ones.
- Number of scripts. A single inbound booking script is simple. Separate scripts for missed calls, web forms, reactivation and reminders each need writing and testing.
- Phone numbers and A2P 10DLC. Business texting in the US to local numbers requires A2P 10DLC brand and campaign registration with the carriers. There are registration fees, monthly campaign fees and approval delays.
- Compliance setup. Consent capture, disclosure wording, opt-out handling and call recording notices. This is where cutting corners gets expensive later.
- Ongoing tuning. Agents improve from real conversations. Someone has to review calls, update the knowledge base and fix the edge cases.
On compliance: AI voice calls and texts should only go to leads who opted in, such as someone who filled out your form or an existing customer who agreed to hear from you. The FCC ruled in February 2024 (FCC 24-17) that AI-generated voices count as "artificial" voices under the TCPA, so those calls need the same prior express consent as other artificial or prerecorded calls. This is not legal advice. Confirm your setup with your own counsel.
Hidden costs nobody quotes you
The invoice is only part of what you spend. These costs show up later and are easy to miss in a pricing comparison.
Your time to build and maintain
On a DIY tool, you are both the developer and the support desk. Put a real hourly value on that time. If an hour of your time is worth more than the software saves, the "cheap" option is the expensive one.
Leads you miss during setup
Every week the agent is not live is a week of leads answered late or not at all. A setup that drags for two months has a cost equal to the appointments you did not book in that window. Executive Stride gets most clients live in 2-3 weeks, and agents are tested in simulated calls before they talk to a real lead.
Weak scripts
A badly written agent costs the same per minute as a good one and books fewer appointments. It also burns leads you paid for in ad spend. Script quality is a large swing factor in cost per booking. Our AI appointment setter script guide covers what a good one looks like.
Slow follow-up
Speed is where the money hides. In an audit of 2,241 US companies, Harvard Business Review found only 37% responded to a web lead within an hour. Firms that reached a lead within an hour were nearly seven times as likely to qualify it as firms that waited an hour longer. A setter who works 9 to 5 cannot reach the 9 p.m. form fill until morning. Our speed to lead service exists for that gap.
The math: cost per booked appointment and cost per show
Monthly price tells you what you spend. Cost per booked appointment and cost per show tell you what you get.
- Add up every monthly cost for the option: software or retainer, usage, phone and texting fees, setup fee spread over 12 months, payroll overhead and your own time at an hourly value.
- Divide by appointments booked that month. That is cost per booked appointment.
- Divide the same total by appointments that showed up. That is cost per show.
- Compare cost per show against what a shown appointment is worth to you (close rate times average job value).
Cost per show matters most, because a booked appointment that no-shows produces nothing. Reminders and confirmations are part of what you are buying.
Hypothetical worked example
The numbers below are made up to show the method. They are not market prices, vendor quotes or Executive Stride pricing.
Say you get 200 leads a month from ads and your website. You compare a DIY tool against a done-for-you agency, then against one full-time human setter. The booking and show counts are assumptions too: replace them with what you see in a real test, since your results depend on your leads, offer and scripts.
- DIY: $500 a month in usage, plus 15 hours of your time valued at $75 an hour ($1,125). It books 55 appointments and 30 show.
- Done-for-you: a $3,000 setup fee spread over 12 months ($250) plus a $1,500 monthly retainer. It responds in seconds around the clock, books 70 appointments and 42 show.
- Human setter: base pay at the BLS receptionist median (about $3,168 a month), 20% for payroll overhead ($634), and a $25 bonus per booked appointment. Working business hours, they book 50 and 30 show.
| Option | Total monthly cost | Booked | Showed | Cost per booked | Cost per show |
|---|---|---|---|---|---|
| DIY software | $1,625 | 55 | 30 | $29.55 | $54.17 |
| Done-for-you agency | $1,750 | 70 | 42 | $25.00 | $41.67 |
| Human setter | $5,052 | 50 | 30 | $101.04 | $168.40 |
In this made-up case the agency costs a little more per month than DIY and less per show, because the example assumes it books more and you are not spending your evenings on it. If your DIY build books as well, DIY wins on cost. Your numbers will differ. Plug in your own lead volume, booking rate and show rate before you decide.
The example also leaves out what the human does well. A good setter handles upset callers and complex questions better than an AI agent. That is why many teams land on a hybrid.
How to get a quote you can compare
When you ask vendors or agencies for pricing, ask the same questions of each so the answers line up.
- What is the setup fee, and what exactly does it include?
- Is usage (minutes, texts, numbers, A2P registration) included or billed on top?
- Which integrations are native, and which are custom work?
- Who writes and tunes the scripts after launch, and how often?
- How does the agent hand a conversation to a human?
- How are consent and disclosure handled for your state and industry?
- What reporting do you get on booked appointments and shows?
If a quote cannot be turned into cost per show, ask until it can. Our done-for-you AI appointment setters page lays out what a full build covers. If you run a trade or local service business, the roofing and home services pages show how the setup fits those lead flows.
FAQ
How much does an AI appointment setter cost?
It depends on how you buy it. DIY tools bill per minute, seat or message plus your build time. Agencies charge a setup fee and monthly retainer. Compare them by cost per booked appointment and cost per show.
How much should I pay an appointment setter?
The closest public benchmark is the BLS receptionist median of $18.27 an hour. Most setters earn a base wage plus a bonus per booked or shown appointment, and you add payroll taxes and management time on top.
Is there an AI appointment setter?
Yes. AI appointment setters are voice and SMS agents that answer opted-in leads, ask qualifying questions and book them on your calendar. Good ones hand off to a human when asked or when a question falls outside what they know.
What hidden costs come with an AI appointment setter?
Your own time on a DIY build is usually the largest, followed by weak scripts. Both raise your cost per booked appointment even when the software bill looks small.
